Renal and circulatory dysfunction in liver cirrhosis. Pathogenesis and treatment

چکیده
Sodium Retention Sodium retention is probably the most common renal function abnormality in patients with cirrhosis and ascites and plays a key role in the pathophysiology of ascites and edema formation. It is the first manifestation of renal impairment cirrhotics develop. It appears before ascites formation, impaired free water excretion and functional renal failure (Figure 1). This observation suggests that sodium retention is a cause and not a consequence of ascites formation in cirrhosis.

Pathogenesis and treatment of hepatorenal syndrome.
Hepatorenal syndrome (HRS) is a functional renal failure that frequently develops in patients with advanced cirrhosis and severe impairment in systemic circulatory function. Traditionally it has been considered to be the consequence of a progression of the splanchnic arterial vasodilation occurring in these patients. However, recent data indicate that a reduction in cardiac output also plays a ...
متن کاملEarly Detection of Renal Dysfunction in β Thalassemia with Focus on Novel Biomarkers
Improved survival among transfusion dependent thalassemia patients in recent years has led to the manifestation of morbidities such as renal dysfunction. Renal injury is still an underestimated complication in β thalassemia major patients. Chronic anemia, iron overload due to repeated transfusion, and specific iron chelators are the main factors in pathogenesis of renal dysfunction in β thalass...

Hepatorenal syndrome (HRS) is a frequent complication in patients with advanced cirrhosis and ascites. It is characterized by an intense renal vasoconstriction, which leads to very low renal perfusion and glomerular filtration rate (GRF). The renal ability to excrete sodium and free water is also severely reduced [1,2]. Renal histology shows no significant lesions sufficient to justify the impa...
متن کاملBINDING OF NAPROXEN TO SERUM PROTEINS IN PATIENTS WITH LIVER CIRRHOSIS AND HEALTHY INDIVIDUALS
A three-fold decrease of the in vitro ability of serum proteins to bind naproxen was shown in patients with cirrhosis of the liver compared to healthy individuals. This decrease was caused by quantitative changes in serum proteins in the sera of patients with liver damage. Lower doses of naproxen are therefore suggested in the treatment of patients with liver dysfunction.
متن کاملThe kidney in chronic liver disease: circulatory abnormalities, renal sodium handling and role of natriuretic peptides.
Patients with chronic liver disease (Läennec's cirrhosis) present sodium chloride retention, leading to fluid accumulation within the extracellular space (edema) and specially in the abdomen (ascites). This article reviews the pathogenesis of the hemodynamic abnormalities observed in these patients, particularly the hypothesis of "primary arterial vasodilation", with an increased nitric oxide p...
